Dean Health Plan and SSM Health believe health care should be affordable and local; Copay Elite plans help bring that to life.

Through our integrated health care delivery network, Copay Elite plans provide excellent health care coverage from local, trusted providers at a lower cost to you. When you or your employees select a primary care location for a Tier 1 copay benefit, your office visit copay is lower than traditional office copays. Copay Elite plans are available in Green Lake, Iowa and Jefferson counties for Small Group and Individual and Family plans, and in Dodge County for Individual and Family plan members.
However, Tier 1 copay benefit plan locations are located throughout our entire 20-county network.
Tier 1 copay benefit services include primary care, optometry, urgent care and physical, occupational and speech therapies. If you are looking for a specialty provider not listed here, visit deancare.com/doctors.
