Employers with 50 or more employees will receive renewal materials 75 days prior to your renewal date.
The following documents will be included:

  • Cover letter providing the date we need to receive plan changes 
  • Explanation of renewal
  • Renewal plan rate table
  • Rate Sheet – complete and return this form. It provides your new rates for the renewal plan and may contain some alternate plan options. You need to complete the acceptance information, including indicating the plan you wish to renew with and updating your contact information. To ensure your premium billing and Summary of Benefits and Coverage distribution are correct, be sure to return the rate sheet by the date indicated in the cover letter.
  • Benefit summaries for all plans quoted
  • Benefit changes (if applicable) 
    • Community-wide benefit changes
    • Legislative mandates  
  • Group information form – Complete and return. This form is used to determine and validate: 
    • Medicare eligibility and requirements
    • COBRA participation and contribution strategies
    • Other carriers offered
  • Plan Brochure(s)

Note: If the rate sheet is not returned by the date indicated, we will renew your group with the current or closest available ACA-compliant plan. Failing to return the rate sheet will not result in terminating your group policy. Group terminations require written notification to your account manager.

Employers with 2 to 49 employees will receive renewal materials 60 days prior to your renewal date.
The following documents will be included:

  • Cover Letter Providing the Date We Need to Receive Plan Changes 
  • Rate Table for the Renewal Plan
  • Rate Sheet – Please Complete and Return

This form provides your new rates for the current or closest available ACA-compliant plan and may contain some alternate plan options. You will need to complete the acceptance information, which includes indicating the plan you wish to renew with and your contact information. To ensure your premium billing and Summary of Benefits and Coverage distribution are correct, be sure to return the Rate Sheet by the date indicated in the cover letter.

This form provides your new rates for the renew-as-is plan and may contain some alternate plan options on the top half of the page. You will need to complete the acceptance information on the bottom half of the page. Indicate if you are renewing on the current plan or making a change, then complete your contact information and sign the Renewal Rate Sheet. To ensure your premium billing and SBC distribution are correct be sure to return the Renewal Rate Sheet by the due date indicated in the cover letter.

Please Note:
Please return the Renewal Rate Sheet in your renewal materials no later than the due date indicated on the cover letter. This will ensure a correct billing statement and correct SBC information is mailed to your insured employees. If this form is not returned by the indicated date, we will renew your group with the renewal plan as indicated on the Renewal Rate Sheet and the billing will reflect those renewal rates and SBCs reflecting the renewal plan will be mailed 30 days prior to the renewal date to the covered subscribers.

 

Need to make changes to your plan or contract?

Your Account Manager can help if you need to make any changes to your plan or contract at renewal time. If you elect to change your benefit plan design, all plan changes need to be requested and approved prior to your renewal or anniversary date.

Please note that benefit upgrades are subject to review and approval.

Changing contract provisions

If you need to change any of your contract provisions, contact your Account Manager. Any approved changes become effective on the first of the month following the date we receive the request. All requests are subject to approval, such as: 

   • New hire probationary period

   • Return from lay-off

   • Employee termination

   • Rehire provision

Business structure or ownership changes

If your group has any ownership or business structure changes – such as mergers or acquisitions – notify your Account Manager immediately. Certain changes may require additional forms to be completed for our records.

You are responsible for notifying us within 10 days of a change in legal status, business expansion or dissolution, merger, buyout or other significant business changes. You must also notify us at least 90 days before additional employee segments are added as they are subject to our approval.
