Dean Advantage Essential (HMO), Assurance (HMO-POS), Balance (HMO-POS), Complete (HMO) and Harmony
The service area includes the following counties in Wisconsin: Columbia, Dane, Dodge, Fond du Lac, Green, Iowa, Jefferson, Rock and Sauk. You must live in one of these counties to be eligible to enroll.

Out-of-network/non-contracted providers aren't obligated to treat Dean Advantage members, except in emergency situations. To find out if we will cover an out-of-network service, you or your provider should call us and ask for a pre-service determination before you receive the service.
