Contact the Dean Health Plan Customer Care Center at (800) 279-1301. We will be happy to mail the Flu Vaccination Reimbursement Form to you. It should arrive in three to five business days. Complete the form, attach your receipt and return it to Dean Health Plan.
 

No. Your annual flu vaccine is a covered benefit, and you do not need a prescription to receive it.

We’ll process the claim within 45 days after receiving the receipt and claim form.

You may only receive one reimbursement per calendar year.

If you receive your flu vaccine at an in-network pharmacy, the pharmacy can process your vaccine claim directly through Dean Health Plan and Navitus. You will have no out-of-pocket cost. If you receive it at a pharmacy outside the network, you will need to pay out of pocket for the vaccine, then submit for reimbursement up to $33.79.

No. Dean Health Plan cannot reimburse flu vaccines paid for with gift cards.

When you see a Dean Health Plan provider and receive your flu shot through a provider's office or clinic, the claim will be submitted by your physician and covered at 100% through your policy with Dean Health Plan.

You can ask for a duplicate receipt from the vaccine administrator. Dean Health Plan will not reimburse you if you do not have a receipt for the flu vaccine.

Yes. You can receive your flu vaccine anywhere, including non-plan providers, and receive the $33.79 reimbursement. If you receive the vaccine from a non-plan provider, simply pay for the service at the time you receive it and send a copy of your receipt with the Flu Vaccination Reimbursement Form.
