Some local exercise facilities offer discounts to our members through the Healthy Partners program. Although you can receive a WIN reimbursement at any health club, you can receive additional discounts at Healthy Partners participating facilities.

Dean Health Plan's Wellness Incentives Now (WIN) Program provides reimbursement for participating in numerous health-related activities such as health club memberships, weight management programs, health education training and more.

With a wide variety of options, you're sure to find activities and complementary services that best fit with you and your family. Anyone who is covered under your DHP insurance is eligible to participate - children and adults alike.

How it Works

We will reimburse individual members up to $100 or families up to $200 for eligible services received per calendar year.

Just follow these easy steps!

Pay for the services at the time it is rendered. Send in your proof of payment—credit card statement, bank statement, cancelled check or receipt—along with the reimbursement form. If you submit a credit card or bank statement, please indicate where the WIN eligible charge appears. Reimbursement for services can take up to 60 days from the date your form is received by Dean Health Plan. You will receive a statement detailing reimbursement or explanation of processing of this form. Due to the high number of WIN forms submitted at the end of 2011, please be aware that some delays will occur in processing your reimbursement.
