Primary Care

Your Partner in Health

Your personal wellness is our priority. We don’t simply treat diseases but strive to help patients stop illness before it starts. As your first point of care, your primary care provider (PCP) manages your routine wellness visits and screenings, cares for you when you have a non-emergency illness and refers you for any specialized care you may need.

Benefits to Having a Primary Care Provider

Our physicians and advanced practice providers take time to listen and learn about you, your family history, and your over-arching health concerns over time. We get to know you as a person, so we can better address what really matters to you as a patient.

Earlier treatment and improved outcomes

Research has shown that yearly preventive care "check-ups" often lead to earlier treatment and improved outcomes. During your yearly wellness exam, your provider will develop a comprehensive care plan offering medical counseling and health monitoring suggestions based on your current health and risk factors. This often includes recommended preventive screenings and simple daily lifestyle changes that could improve the overall quality and longevity of your life.

Convenient care when you need it most

You can contact your PCP conveniently with MyChart online or through the mobile app. Additionally, you can schedule in person and online appointments (where available), message with your provider, request prescription refills, and more with secure 24/7 access.

Direct referrals to the best specialists

Your primary care provider will act as your “point person” connecting you with our vast network of award-winning SSM Health specialists, surgeons, nutritionists, and mental health professionals in the region.

Finding Your Fit

Your Primary Care Medical Team

When choosing a primary care provider, you will notice a few different titles. Our care team works together in varying capacities to provide you exceptional care.

Physicians

Physicians can be licensed as either doctors of medicine (M.D.) or doctors of osteopathic medicine (D.O.) The difference is the type of medical school they attended. Both are fully licensed and trained doctors who provide a full range of services and have many educational routes and practice similarities. Both also go though the same type of residency or fellowship training.

Residents and Fellows

During your visit, you might speak to or receive treatment from a resident or a fellow. A fellow is a physician who has finished their residency training and is pursuing additional training in a specialty. They can prescribe treatment independently.

A resident is a physician who has graduated from medical school and wishes to specialize prior to starting an independent practice. They treat patients, but will often consult with a residency-trained physician within the office prior to completing a visit.

Advanced Practice Providers

Many patients choose an advanced practice provider as their designated primary care advocate. If your doctor happens to be out, you can also often receive medical advice or treatment from advanced practice providers on the team.

Physician assistants (PAs) practice medicine on a team and are supervised by physicians. PAs can diagnose a patient, prescribe treatment, and perform advance procedures.

Nurse practitioners (NPs) are advanced practice registered nurses who have advanced clinical training. NPs can diagnose a patient, prescribe treatment, and perform advanced procedures.

Clinic Support Staff

All primary care locations have a team of clinical support staff to hear your concerns, collect vitals, record medical history, and coordinate your care. Clinic nurses can have a variety of titles from Registered Nurses (RN) and Medical Assistants (MA) and work seamlessly together to provide exceptional, compassionate care.
