    • ChefsFor businesses that have two to 99 insured employees and located in Dean Health Plan's service area.

      We offer a wide variety of benefit options with deductibles ranging from $0 to $5,000 and multiple options for co-insurance, co-pays and prescription drug benefits. In certain situations more than one benefit plan may be offered to employees. In addition, Dean Health Plan offers plans that allow you to use any provider network, even for employees who live outside the Dean Health Plan service area.

    • • Complete a Dean Health Plan Employer Group Application
      • Submit the following to Dean Health Plan
         - Employee Applications
         - Most recent Quarterly Wage & Tax Statement
         - Most recent health insurance premium bill (if applicable)
