Q. How can I get a reimbursement form for someone who does not have access to a PC?
A. Contact the Customer Care Center at (800) 279-1301. We will be happy to mail the reimbursement form to you. It should arrive in three to five business days. Please complete the form, attach your receipt and return it to Dean Health Plan.

Q. Do I need a prescription from my doctor to get this shot?
A. No. The routine flu shot is a covered benefit, and you do not need a prescription to receive it.

Q. How long will it take to receive reimbursement?
A. Dean Health Plan will process the claim within 45 days after receiving the receipt and claim form.

Q. Is there a certain time frame in which I can get the shot?
A. You may only receive one reimbursement per calendar year.

Q. How much will I pay if I receive the vaccine at a pharmacy?
A. If you receive your flu vaccine at a Dean Clinic pharmacy, the Dean Clinic pharmacy is able to process your vaccine claim directly through Dean Health Plan and Navitus. You will have no out-of-pocket cost. If you receive it at a pharmacy outside the network, you will need to pay out of pocket for the vaccine, then submit for reimbursement of up to $27.

Q. Can I be reimbursed for immunizations paid for with a flu shot gift card?
A. No.  Dean Health Plan cannot reimburse flu shots  paid for with flu shot gift cards.

Q. If I get the flu shot at a Dean Clinic or from a Dean Plan provider, will I still need to pay out-of-pocket first and be reimbursed later?
A. When you see a Dean provider or Dean Plan provider and receive your flu shot through a provider's office or clinic, the claim will be submitted by your physician and covered at 100% through your policy with Dean Health Plan.

Q. What are my options if I lose or can't find my receipt?
A. You can ask for a duplicate receipt from the vaccine administrator. Dean Health Plan will not reimburse you if you do not have a receipt for the flu vaccine.

Q. When you say the shot can be received anywhere, does this include at work and with non-plan providers?
A. Yes. You can receive your flu vaccine anywhere, including non-plan providers, and receive the $27 reimbursement. If you receive the vaccine from a non-plan provider, simply pay for the service at the time you receive it and send a copy of your receipt with the reimbursement form.
