WIN (Wellness Incentives Now!) Program

Dean Health Plan's WIN (Wellness Incentives Now!)
Program offers cash back for a variety of health-related expenses
for the whole family. Anyone who is covered under Dean Health
Plan insurance plan is eligible to participate, children and
adults alike.

Benefits and Reimbursement
Like the rest of your health coverage, the WIN program runs on a contract year. WIN Program eligible services must be obtained during that contract year.
The deadline for submitting a WIN reimbursement form is 90 days following the end of your contract year.
Individuals
Get up to $100 back! Dean will reimburse
individual members for eligible services received, up to $100
back each contract year.
Families
In addition to the individual maximum, there is also a maximum reimbursement
limit of $300 per family, per contract year.
Bonus $100 Weight Management Reimbursement
By participating in
any of Dean
Health System Comprehensive Weight Management non-surgical
programs, members will receive an additional $100 bonus reimbursement.
This is an extra reimbursement that does not count toward the
$100 individual or $300 family
WIN reimbursement maximum.
How and When Do I Receive Reimbursement?
1. You pay for the service at the time it is rendered.
2.
Then send in your receipt
along with the WIN
Program Reimbursement Form to Dean Health Plan.
3.
Within 30 days we will
send you a check as a reward for your healthy endeavor!
As you and your family receive eligible services, send in your form and receipt, and we will send your reimbursement each time until you reach the reimbursement maximum.
WIN Program Eligible Services
Complementary Health Services
Acupuncture, massage therapy and other complementary health therapies
performed by a licensed acupuncturist, therapist or provider.
(Their license number must be submitted with your reimbursement
form.)
Weight Management
Includes membership dues or enrollment fees for weight management
programs. Examples include Weight Watchers, Jenny Craig, and
Dean's Comprehensive Weight Management Program (see
bonus reimbursement information above.) Internet-based
programs are not included at this time.
Health Education
Health and wellness education classes, and lifestyle coaching.
Fitness Focus
Any health club membership or exercise facility fee*, including
exercise classes or martial arts classes.
* Enjoy additional discounts with our Healthy
Partners Program.
Family Focus
Any youth recreation programs, community pool memberships, birthing
classes, breast pumps, babysitting or safety classes (including
CPR).
Community Focus
Registration fees for walk, run or bike events and community
recreation programs.
Healthy Food Focus
Includes community supported agriculture (CSA) membership fees.
Members support local farms by paying an annual fee and get
a "share" of the season's harvest of fresh produce. Visit www.macsac.org
