2007 Complementary Health Program

Beginning January 1, 2008, this program was replaced by our WIN program. You may submit a reimbursement for services received in 2007 until March 31, 2008

Dean Health Plan is committed to helping you feel better and we encourage members to take advantage of our Complementary Health Program.

Benefit and Reimbursement for the 2007 Plan Year

Dean Health Plan will reimburse 50 percent of the first $200 in qualified charges, with a maximum reimbursement of $100 per member, per calendar year. All Dean Health Plan members over the age of 18 are eligible to participate.

How and when do I receive reimbursement for the 2007 program?

To receive payment, send in a Complementary Health Program Reimbursement Form (PDF) along with a service receipt and a completed 2007 Exercise Log (PDF) if required. We will reimburse the member directly, not the service provider.

Members have until March 31 of the following year in which services were received to submit a request for reimbursement. For 2007, reimbursement requests must be received by March 31, 2008.
