Assess your employee’s needs and interests

A qualified and trained professional will:

  • Introduce the eight dimensions of wellness
  • ​Use a solution-focused brief intervention to create an individualized strengths based plan
  • Assess and address barriers and social determinants of health connecting to appropriate tools and resources
  • Assist with establishing or connecting with their primary care provider
  • Review their EPIC medical record for any needed preventive health screenings (e.g., colonoscopy, mammogram, immunizations)
  • Help with system navigation and assist with arranging follow-up care, as needed

Following the screening, participants will be guided to take the online health assessment and provided with an individual report card with resources, support, and referrals. The individual screening data combined with a questionnaire about lifestyle behaviors are summarized in an employer aggregate report which guides the overall wellness programming.

  • Assess risk factors, including health history, blood pressure, body mass index, and waist circumference. If risks are identified, non-fasting cholesterol and glucose may be measured via a finger stick. Results, explanation, and education are provided.
  • Assist with establishing or connecting with their primary care provider.
  • Assess and address barriers and social determinants of health connecting to appropriate tools and resources.
  • Review their EPIC medical record for any needed preventive health screenings (e.g., colonoscopy, mammogram, immunizations).
  • Help with system navigation and assist with arranging follow-up care, as needed.

Our staff is trained to provide the National Alliance of Mental Illness' one-hour stigma-free company program. The program's goal is for leaders and employees to support colleagues who may be struggling or have some mental health concerns and be a safe space for people to go to talk or get resources. Part of the training includes having the group develop ideas on how to create a stigma-free culture in the workplace and discuss action steps to implement their plan.
Our team is trained in Critical Incident Stress Management. If your company or employees experience a traumatic event (e.g. unexpected employee death, an act of violence, or something catastrophic), Critical Incident Stress Management is a formal process that is voluntary for helping those involved. Critical Incident Stress Management allows them to share their experiences, vent emotions, learn about stress reactions and symptoms, and receive referrals for further help including referral to therapy and other services as needed.
