Medical Records / Frequently Asked Questions
How can I get copies of my medical
record and will there be a charge?
Dean Health System requires that a Patient Authorization

to the clinic that provided the service. There is no charge for medical records
that are released to another healthcare provider. Dean Health System may charge
a reasonable fee if the medical records are for personal use.
Can I view my medical
record and will there be a charge?
Patients can view their medical record by setting up an appointment with the
Release of Information Specialist at (608) 252-7784. Viewing appointments are
scheduled for one hour. If copies are requested, Dean Health System may charge
a reasonable fee to fulfill this request.
Can I get copies of my children's
