Dean Health Plan's Chamber Health Insurance Program provides Chamber member businesses with comprehensive and affordable insurance options. Products are available for sole proprietors and businesses that offer coverage for their employees.

When a business participates in the Chamber Health Insurance Program they are partnering with other local businesses to help keep renewal costs manageable.

Dean Health Plan offers eligible chamber of commerce members exclusive savings on our individual health insurance plans. Eligible applicants must be an individual member or employee of a group membership with a Dean partnering chamber of commerce. We offer a discount on monthly premium rates as well as multiple plan options to fit your budget.

Group Plan Options

ChefsFor businesses that have two to 99 insured employees and located in Dean Health Plan's service area.

We offer a wide variety of benefit options with deductibles ranging from $0 to $5,000 and multiple options for co-insurance, co-pays and prescription drug benefits. In certain situations more than one benefit plan may be offered to employees. In addition, Dean Health Plan offers plans that allow you to use any provider network, even for employees who live outside the Dean Health Plan service area.

How to Apply

• Complete a Dean Health Plan Employer Group Application
• Submit the following to Dean Health Plan
   - Employee Applications
   - Most recent Quarterly Wage & Tax Statement
   - Most recent health insurance premium bill (if applicable)
