Healthcare Decisions Day 2017 

​It’s never too early to think about what you want to happen if you become ill and can’t speak for yourself. Do you know who you would want speaking for you and making decisions on your behalf?
Sunday, April 16 is National Healthcare Decisions Day – a day set aside to educate the public and providers about the importance of advance care planning. SSM Health will observe the day Tuesday, April 18, when you will have the opportunity to stop by one of seven locations to complete a healthcare power of attorney (HCPOA) form. This document allows you to make your wishes known, and appoint two family members or friends 18 years or older to make healthcare decisions for you if two doctors determine you can no longer decide for yourself.
Why is this important? Wisconsin is not a "next of kin" or "family consent" state. This means that if you don't have a HCPOA naming a decision maker, your family may have to pursue legal guardianship through the courts to make medical decisions for you if you become incapacitated.
Join us to fill out your health care power of attorney form! Tables will be set up for staff, physicians, volunteers and visitors to complete HCPOA forms. Social workers will be available to answer questions, provide information and help with filling out the document. No registration is necessary.                  
                                                                                                                Events will be held at these locations throughout the Wisconsin Region:
     • SSM Health Dean Medical Group, 1821 S. Stoughton Rd: 10 am to 2 pm, Amethyst Room
     • SSM Health Dean Medical Group, 1313 Fish Hatchery Rd: 10 am to 2 pm, Team Site Room (an interpreter will be available at this location for Spanish speakers)
     • SSM Health Dean Medical Group, 752 N. High Point Rd: 10 am to 2 pm, Prairie Room
     • SSM Health St. Mary’s Hospital - Madison from 11 am to 1:30 pm, Kiosk
     • SSM Health Outpatient Center: 10 am to 2 pm, third floor Neurology
     • SSM Health St. Clare Hospital - Baraboo: 9 am to noon, main lobby
     • SSM Health St. Mary’s Hospital - Janesville: 10 am to 2 pm, Town Square
