Transfer Your Medical Records

If you've chosen a Dean Clinic doctor and called the clinic to make an appointment, have your medical records sent over from your old clinic by filling out our Authorization to Release Protected Health Information form. If you've chosen a provider outside of Dean Clinic, you will need to find their form on their website.
