New health transformation consultants join Dean Health Plan

The health promotion team is expanding to meet the needs of our members

Jessa Kinnamon and Sean Ward are two new Health Transformation Consultants (HTC) joining Dean Health Plan and the Health Promotions team. They play an essential role in taking wellness programs to the next level.

Through Personal Wellness Plans, HTCs focus on holistic health – mind, and body – and pay attention to what might be getting in the way of a member living their best life. While HTCs guide the conversation, it's all member-led. They understand a member's social determinants of health, conduct active listening and solution-focused interventions and connect them to the resources they need, whether that means counseling, meeting with a provider, or accessing resource kits.

Originally from Milwaukee, Sean is a UW and SSM Health alumnus. He has over a decade of experience in social work and case management in acute care settings. In addition, Sean has over ten years of experience working directly with clients in holistic wellness, teaching meditation, yoga, mindfulness, and energy healing. Sean's service mindset and the sum of his experiences complement our whole-health approach to wellness. 

Jessa has a BS in Elementary Education from UW–Madison and a Masters in Curriculum and Instruction from the University of Colorado. Jessa was an educator, mentor, and professional trainer for a local school district for nearly ten years, where she used a trauma-informed approach to instruct students and teachers. Jessa has two college-age children and enjoys year-round outdoor activities with her husband and dog. Her experience in training and mentoring will be a great new addition to our services.

You'll see our HTCs in a Personal Wellness Plan screening, Freedom from Smoking groups, NAMI Stigma-Free Trainings, Critical Incident Stress Management training and book clubs.
