Dean Medicare Advantage eligibility

You may be eligible for a Dean Advantage plan if you:

  • Are a Medicare beneficiary (learn more about Medicare)
  • Have Medicare Part A and Part B (and continue to pay your Part B premium)
  • Reside in Columbia, Dane, Dodge, Iowa, Jefferson, Rock or Sauk counties
  • Do not have end-stage renal disease (some exceptions may apply; please speak with a Dean Advantage Sales Representative)

Do you get extra help for prescription costs?

If you receive extra help from Medicare to pay your Medicare prescription drug plan costs, your monthly Dean Advantage premium will be reduced. Find out more

Choose this plan if you prefer to:

  • see the dedicated doctors in the Dean network.
  • have predictable copays and the lowest monthly premium possible under Dean Advantage.
  • keep your monthly expenses low, and are comfortable with a higher maximum out-of-pocket.
  • have prescription drug coverage with manageable copays, but anticipate relatively few and/or inexpensive prescription drug needs.

Choose this plan if you prefer to:

  • see the dedicated doctors in the Dean network.
  • want greater benefits and lower copays at each office visit, and you’re willing to pay a little more in monthly premium in exchange.
  • pay fewer out-of-pocket expenses, and you’re willing to pay extra in monthly premium.
  • have prescription drug coverage with manageable copays and a lower deductible on applicable drugs.

Choose this plan if you prefer to:

  • see the dedicated doctors in the Dean network, but when you travel you want out-of-network care coverage if necessary.
  • have the greatest benefits Dean Advantage offers, along with only low and no copays each visit.
  • pay no more than $2,000 in-network and $10,000 out-of-network.
  • want prescription drug coverage with manageable copays and no deductible on applicable drugs, and you anticipate a great need for prescription drugs.

Dean Advantage is an HMO/HMO-POS plan with a Medicare contract. Enrollment in Dean Advantage depends on contract renewal.

This information is not a complete description of benefits. Contact the plan for more information. Limitations, copayments and restrictions may apply. You must continue to pay your Medicare Part B premium. Benefits, premiums and/or copayments/coinsurance may change on January 1 of each year. The Formulary, pharmacy network, and/or provider network may change at any time. You will receive notice when necessary. We also list pharmacies that are in our network but are outside the Dean Advantage service area.
